When a recipe calls for canned cream soup, what can I use to replace it that is less processed and lower in sodium?

Instead of using canned cream soup, you can make your own "white sauce" or Bechamel Saucefrom flour and milk, and flavor it with whatever the flavorings in the soup are. For example, if the recipe calls for canned cream of mushroom, make a white sauce with mushrooms.
